Appointments
Office visits are by appointment only Monday through Friday. Please schedule your appointment as far ahead as possible to allow for a better choice of day and time. If requesting an appointment via email, please specify a day and time range that you are available to come in.
Please let us know the type of exam you wish to schedule. For example: follow up, yearly, company physical or insurance exam. It is very important that you register with your full name as listed on your insurance card and have your insurance card available to be copied at your arrival time. Please notify us of all insurance or address changes. For your convenience, you can complete your registration forms online. Please print them off and bring them with you to your appointment.
If you are unable to meet your scheduled appointment time, please call the office as soon as possible for rescheduling. Patients should arrive 15 minutes before their scheduled appointment. Late arrivals (in excess of 15 minutes past appointment time) may be asked to reschedule. We know that your time is valuable, and we will make every effort to honor all commitments.

Filing with Health Insurance Companies
We must have a copy of your insurance card to file any insurance for you or your family member. All insurance changes must be given to us at the time of service. If your insurance coverage changes and we are not notified, you will be responsible for the charges.
Your insurance company requires our office to collect co-payments, deductibles, and co-insurance amounts at the time of service. If you are unable to make your payment according to the terms of your insurance policy at time of check-in, you may be asked to reschedule your appointment. In the event your insurance carrier does not cover certain services, you will be responsible for any unpaid balance.

Prescriptions
When calling for new prescriptions, please have your insurance information available. It is always helpful to know the medications you are taking and allergies. Keep a list, if needed.
When requesting refills, by phone or via email, the following information is needed:
- Pharmacy name and phone number
- Medicine, strength and directions
- The office must be called by 3:30pm, or the prescription may not be called in until the following day.
We ask that request for refills on routine medications be made during regularbusiness hours. Physicians on call are unfamiliar with your medical history.
Clinic Services
Professional Services and Scheduling
Except in cases of an emergent nature, please allow an hour for a routine office visit, or on the initial visit a complete history and physical examination may take up to two hours time. Such examinations include the entire body and are not limited to the apparent affected area.
A Southview physician/nurse is always on call and available to help you with urgent and emergency problems. After office hours, on holidays and weekends, the physician/nurse on call can be reached by calling our physician’s office phone number. Prescription refills and non-urgent problems should be handled during routine business hours and no narcotics or sedatives will be called in after hours.
Diagnostic Services Provided
Our Laboratory and X-Ray departments are certified by the State of Alabama and meet all Federal guidelines. The Commission of Office Laboratory Accreditation (COLA) accredits our Laboratory, insuring the highest standards of quality testing and results for our patients and physicians.
Some specialized laboratory tests are performed by outside laboratories and may require several days processing which may delay reporting to our patients.
All X-Ray films are read by board certified Radiologist. This process may delay reports but it insures the highest quality results. Please allow 7 days to communicate normal lab/x-ray and diagnostic results. In the event of abnormal results, you will be contacted by telephone.
Our X-Ray Department provides regular x-rays, CT scans, Bone Density scans, Ultra sounds and Segmental pressures.
For your convenience the Laboratory and X-Ray Department hours are 7:30am to 4:30pm Monday through Friday. All patients must register at the front desk before any services are rendered.
